    Welcome to this Forum.

    It's a nice chart to a lovely tune. Thanks much for posting it.

    A few comments. On the recording, the head has four extra bars after the first ending that are not in the chart. The tempo is set at 100, but the recording is closer to 180. The first Coda sign should be at the end of the repeat bar of Gb∆7 (right now it's at the beginning of the next bar, G-11), and there should be another Coda sign at the beginning of the last line.

    The whole form is difficult to notate on one page, as the Intro only repeats after the first head and before the last head, not during the solos, and the four-bar extension after the first ending only occurs on the head.

    HEAD: A, B, Extension, A, C
    INTRO

    SOLOS: ||: A, B, A, C :||

    INTRO
    HEAD: A, B, Extension, A, C with CODA

    However, one can fit 192 one-beat bars on a page, and this tune can be notated in 177 bars, so it is possible.

    Do you have a source for a lead sheet?

    It's such a lovely tune that I couldn't resist making a chart that covers the whole form.

    Because each bar is too short to accommodate complex chords and still be readable, I put the extended chords above as Alternate Chords.

    It's not beautiful to look at, but it's playable. Here's the YouTube performance:
