Footprints, by Wayne Shorter.
This arrangement includes an Intro and Outro, and uses the vamp bassline for the head. Its time is the same as Shorter's own recording, at 138 bps, and — per The Standards Real Book, published by Chuck Sher — it's in 6/4 rather than 3/4 time.

Jer's chart is correct. The bass plays an Eb, not a D (the piano is playing the D in the chord which creates that tension between the D and the Eb)
However, The E7 is not altered. It's actually a E13#11 (same as the F chord before). Only the A7 is altered.
The F#-7b5 is correct but using a A-69/F# instead produces a voicing more similar to what Herbie is playing (like a half diminished with a 11th). A CMaj7#11/A could work as well.
