This unusual tune is on pp 107-8 in Sher's New Real Book, Volume Two. The book has it in 6/8, but I think it works better in 4/4.
It doesn't match any of the versions on YouTube. The one I prefer is here.
I aded a couple of passing chords that I thought were appropriate.
